Obamacare for Expats?

The short answer:  “Affordable Healthcare Act does not apply abroad".  That's a headline I read that seems to be confirmed as I keep looking into the subject.

But what qualifies one as 'living abroad'?  I have read somewhere that one would have to be able to show (via passport) that they spent 330 days (out of a year's 365 days) outside of the US.  US citizens living abroad can be assumed to have health insurance in their primary living area.  (Honestly, health care is so inexpensive here that it seems more cost-effective to just pay-as-we-go.)
